Assisted Reproduction Fees

We will bill for in network insurance when services are covered by insurance. Out-of-network and non-billable services are charged as a fee-for-service. We can provide a superbill statement for out-of-network policies or FSA/HSA benefits, but cannot guarantee approval. All teleheatlh services require completion of a credit card authorization form.
